Forms

These forms can be viewed as either Adobe Acrobat .PDF files or Microsoft Word .DOC files. While they can be opened from your web browser, we recommend you save a copy on your own computer and use the forms from there. 

1. To save a copy of the file on your own computer, right-click on the link for the version you wish to save.
2. Choose the option "Save target as..." Be sure to remember where on your computer you saved the file.
3. If you prefer to use Acrobat, version 5.0 or later is required. Please click on the icon below for a free download of the program.
4. Because Form C cards are two-sided forms with return postage paid, they should still be picked up at the AA/EEO Department.   For more information, call 844-4794.
